Why am I being billed twice what my contract says?

I initated a new contract on 19 September at a rate of 59.99.  I am still being billed 84.99 per month.

 

I have called customer service twice, and they said my order "didn't go through" and did another order for me.

 

Nothing happened.

 

Now I have three orders outstanding and Verizon is still charging me 84.99.  Who do I have to talk to if I want something done?
